Each miR Sentinel™ Prostate Cancer Test requires a requisition form to be filled out for the patient. The requisition form will either be included in the Collection Kit shipped to you or provided by our customer service team.  You, as the patient’s provider, will fill out and sign the requisition form, to confirm that the test was properly ordered. The completed and signed form must accompany the sample that is returned to our lab within the same Collection Kit.
