What will trigger a sample rejection by the lab?
The following issues will trigger a sample rejection by the lab:
- Volume of urine provided is less than 30mL.
- The urine sample is not in the cup.
- The cup is not sealed.
- The Collection Kit is not registered or the ID sticker is not filled out and affixed to your sample cup.
- The Collection Kit is mailed on Friday, Saturday, or Sunday.
- The sample is returned in an improper shipping environment e.g., shipped without ice packs, or the ice packs are fully melted and warm upon receipt.
- Sample types other than urine are provided. (Such as peripheral blood, stool, Cerebrospinal Fluid, etc.)
- The samples arrive without a requisition form or the requisition form is incomplete and/or are the samples are mislabeled/unlabeled.
- Information written on the urine cup does not match the Test Request Form
- The sample and or TRF is missing 2 unique patient identifiers (i.e. first name, last name, DOB, accession ID)
- The sample and/or requisition form is not marked with date and time of collection.
- Signs of major leakage or damage to the Collection Kit.
- The shipment is shipped greater than 72 hours following collection.
- The sample is received 7 days post collection
- The sample is contaminated with foreign substances. Foreign substances include but are not limited to, stool, hair, and/or non-biological substances.
- Without a licensed healthcare provider’s signature.
